The Air Suvidha 2.0 health self-declaration form is now mandatory for all international passengers arriving in India.
Reintroduced due to global Ebola surveillance, it requires travelers to disclose their 21-day travel history, exposure, and symptoms prior to entry. Travelers must submit the form online within 24 hours of scheduled departure to India.
The form mandate applies to all passengers (both Indian and foreign nationals). Travelers are required to complete the 2.0 form before boarding.
When filling out the form, you will need your passport, flight itinerary, travel history, and contact details.
