Send e-mail to inpayrol@lehigh.edu to request the following:

  • Copies of Payroll Management Reports
  • Assignment/Appointment Form reprints
  • Departmental Time Report copies or reprints
  • Add or remove Payroll Management Report Recipients
  • Reprints of W-2, 1042S, 1099R
  • Change in Mailing Address

 

University Tax Form/Information Requests

  • 990 Return of Organization Exempt from Income Tax
  • 990-T Exempt Organization Business Income Tax
  • W-9 Request for Taxpayer Identification Number and Certification - click for form
  • IRS 501(c)(3) determination letter
  • Out-of-state information
