Graduate Student Forms
The Bursar's office is in the process of converting our forms to DocuSign, which allows you to complete and sign your documents electronically, with no need to mail, fax or scan. You will receive an email confirmation when the document is complete and all signatures have been obtained. If you do not receive your DocuSign notification via email, please contact our office so we can verify the email address and make any necessary correction to the document routing.
DocuSign forms for Graduate students must be completed by the student. Be sure to use your Lehigh email address when completing the DocuSign routing information.
